Job Summary
The Oncology Clinical Pharmacist Specialist is responsible for the development, implementation and documentation of chemotherapy services in the inpatient and outpatient settings. The pharmacist coordinates the oncology service according to the latest guidelines, assists with clinical pathways, protocols and policies, and provides education to staff.
Responsibilities- Participate in patient‑focused services and provide pharmaceutical care to patients receiving chemotherapy.
- Evaluate chemotherapy regimens for appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and drug interactions.
- Discuss medication order changes or clarifications with prescribers, document changes and inform others.
- Provide patient consultations for chemotherapy and drug information.
- Detect, monitor, document and report adverse drug reactions and medication errors.
- Direct and initiate the development, implementation and maintenance of clinical pharmacy services.
- Provide clinical consultation to medical staff and other providers within the appropriate team or unit, suggesting cost‑effective therapeutic alternatives when needed.
- Communicate and document all activities and interventions (medication order changes, pharmacokinetic dosing/monitoring, adverse drug reactions, errors etc.).
- Collaborate with hospital and pharmacy staff to resolve problems and questions and provide ongoing education.
- Develop educational materials and provide in‑service training for staff.
- Develop interventional strategies to optimize chemotherapy utilization, improve patient outcomes through oncology guidelines, clinical pathways, restriction criteria, formulary management and new laboratory methods.
- Perform formulary evaluation and chemotherapy class reviews for anti‑neoplastic agents and present findings to the Pharmacy & Therapeutics committee.
- Participate on hematology/oncology committees, pharmacy therapeutics committee, and other appointed committees as needed.
- Act as a liaison to other healthcare practitioners, fostering good working relationships with physicians, nurses and other professionals.
- Pharm.
D. with a PGY‑1 residency program. - PGY‑2 in oncology preferred.
- 2–3 years of inpatient pharmacy hospital/clinical capacity experience.
- Currently licensed or registered to practice pharmacy in Louisiana.
- Board‑certified oncology pharmacist (BCOP) preferred.
Variable hours (United States of America).
