Physical Therapist - Full Time
Bridge Home Health & Hospice is seeking a dedicated Physical Therapist to join a compassionate, results‑driven team. The role focuses on delivering exceptional patient care, improving mobility, and supporting families across California.
Experience with per‑visit compensation and a commitment to continuous learning are essential.
Location:
Full‑time, Branded in Merced/Stanislaus/Madera/Los Banos Counties.
Base pay: $75.00/hr – $212.50/hr
Paid per visit: $75 – $212.50 per visit.
- Provide physical therapy services according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
- Assess and evaluate therapeutic/rehabilitative/functional status; participate in development of the total plan of care; evaluate home environment.
- Assess muscle strength, mobility, gait, ROM and potential for rehab.
- Direct physical therapy treatment.
- Instruct patients and families/caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy.
- Report to physician patient’s reaction to treatment or changes in condition.
- Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
- Train patients in the use of prosthetic devices.
- Identify patient and family/caregiver needs for other home health services and refer as necessary.
- Prepare and submit clinical and progress summaries based on attainment of goals.
- Participate in discharge planning for patients.
- Provide physical therapy consultation to home families/caregivers when indicated.
- Provide in‑service education programs for nursing organization personnel as needed.
- Participate in peer consultation process.
- Supervise physical therapy assistants according to organization policy and state regulations.
- When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two (2) weeks.
- Adhere to the company attendance and reliability guidelines, which are an essential function of the job to ensure quality, and consistent patient care.
- Post baccalaureate degree in physical therapy (Doctor of Physical Therapy) approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
- Licensed to practice as physical therapist by the Physical Therapy Board of California.
- Good verbal and written communication and organization skills.
- Current CPR certification.
- Current Auto Insurance and valid California Driver’s License.
- Licensed driver with automobile in good working order and insured in accordance with organization requirements.
- Sit occasionally for 1‑3 hours.
- Stand frequently for 3‑6 hours.
- Drive/operate foot control occasionally for 1‑3 hours.
- Walk occasionally on level surface for 1‑3 hours.
