Job SummaryThe Case Manager Generalist provides case management services to clients classified as low acuity. The Case Manager Generalist is the primary point of contact to coordinate services and care for assigned clients. Each Case Manager Generalist handles a caseload of individuals in need of a wide variety of human services resources and provides the full range of services available.
Pay Rate$22.91 - $24.12 Per Hour

Duties and Responsibilities- Conduct comprehensive screenings and assessments with clients to collect functional, environmental, psycho‑social, financial, employment, housing, educational and health information.
- Assess barriers, develop case plans, coordinate individualized planning and provide service linkages to meet short‑and long‑term needs.
- Coordinate case conferences, consult with service team, monitor progress, and assist clients in meeting agreed‑upon goals.
- Advocate for clients before judicial, community and administrative bodies and facilitate support groups and client participation as appropriate.
- Must be able to pass fingerprint clearance, background check and verifications.
- High School / GED. Completion of VOALA Case Management training within 6 months of hire.
- Minimum 1‑year general experience in case management or similar social services role.
- Bachelor’s Degree in Social Work, Social Services or related field provides alternative to experience.
- Bachelor’s Degree in Social Work, Social Services, public administration, public health or related field.
- 1 year of experience working with target population.
- HMIS navigation.
