POSITION OVERVIEW

Vista Del Mar UAC and Family Services-Vista Safe Homes HSPRS is seeking a dedicated and compassionate Post-Release Service Intensive Level 3 Case Manager (PRS Intensive Level 3 Case Manager) who is a flexible thinker and takes initiative to independently problem-solve in crisis situations. In this crucial role, you will be responsible for providing comprehensive case management services to unaccompanied alien children (UAC) who have been released from federal custody and placed with approved sponsors.

The intensive nature of Level 3 post release services reflects the UAC’s higher level of interventions needed to ensure their safety and stabilize the UAC in the home and within their community. Culturally and linguistically appropriate interventions and supportive services are provided to the UAC and the families they are placed with through weekly in-person contacts for the first 45 to 60 days depending on needs(s).

Following the end of weekly contacts, monthly contacts will be provided for 6 months from date of acceptance. PRS Intensive Level Case Managers will carry a minimum caseload of (3) Level 3, (3) Level 2 and (3) Level 1 PRS cases.

Essential Job Duties(other duties may be assigned):

  • Assessment and Planning: Conduct weekly and then monthly in-person, thorough assessments of each unaccompanied UAC’s needs, strengths, and challenges. The in-person assessments are crucial for determining the UAC’s affect and ability to be a reliable self-reporter. This clinical assessment approach is part of ongoing efforts to build a viable support system, reliable communication conduits and constructive collaboration with the UAC, sponsor, and other household members.
  • Crisis Intervention: Be prepared to respond to crises and emergencies affecting UAC, working in collaboration with emergency services and appropriate agencies as necessary. PRS Intensive Level 3 Case Manager will regularly interact with UAC Protection Services, Trafficking Victims Assistance Program and behavioral healthcare providers.
  • Case Management: Provide crisis-intervention and ongoing case management services to, including assistance with accessing education, medical care, mental health services, legal assistance, and other essential resources. Conduct UAC centered needs assessments, develops individualized strength-based, goal-oriented service plans, and monitors progress of goal completion.
