Position: Case Manager - Permanent Supportive Housing

Case Manager - Permanent Supportive Housing

Position Summary

The Permanent Supportive Housing Case Manager provides intensive case management to formerly unhoused Transition Age Youth (TAY) and families with complex needs, including chronic medical conditions, mental health, and substance use disorders. Using practices such as Motivational Interviewing (MI), Critical Time Intervention (CTI), and Stages of Change (SoC), the Case Manager supports clients in building stability, independence, and long-term housing retention.

Responsibilities
  • Manage a diverse caseload of TAY residents with significant barriers, including being unhoused, trauma, and co-occurring disorders.
  • Document case notes and resident progress in CHAMP and HMIS databases.
  • Conduct assessments, create individualized service and goal plans, and provide ongoing support.
  • Maintain consistent contact with clients through unit visits, check-ins, and crisis intervention.
  • Facilitate supportive services, including life skills groups, wellness activities, and referrals.
  • Assist with universal housing applications and coordination through CES, FSC, and other systems.
  • Collaborate with property management and participate in team meetings and trainings.
  • Present resident case reviews to the Senior Housing Program Manager on a bi-weekly basis.
  • Provide coverage for evening/weekend programs as needed and transport clients in the UBH van to various agencies such as LACDMH, DMV, SSA, or other support group meetings.
  • Report abuse as a mandated reporter, including immediately reporting concerns regarding rights violations or suspected abuse.
  • Uphold agency standards for safety, confidentiality, and professionalism.
  • Generate monthly outcome reports and other pertinent reports as requested.
  • Other duties as assigned.

Qualifications
  • Bachelor’s degree in Social Work, Psychology, Sociology, or related field.
  • At least 2 years of case management or advocacy with homeless or mentally ill populations.
  • Familiarity with DHS documentation and client databases (CHAMP/HMIS preferred).
  • Ability to provide non-judgmental support and direction to individuals and families in stressful circumstances.
  • Strong organizational, communication, and crisis management skills.
  • Valid California driver’s license and access to an insured vehicle.

Work Environment

The employee may be in contact with families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health, and safety practices. The employee may experience several unpleasant sensory demands associated with the participant’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crises.
