About Upward Bound House (UBH)

Upward Bound House is dedicated to ending family homelessness in Los Angeles. With over 30 years of experience, we provide a continuum of housing and support services that help families and young parents transition from crisis to stability. As we launch our new DMH-funded interim housing program, we’re seeking passionate, experienced professionals to join our growing team.

Position Summary

The DMH Case Manager will deliver trauma-informed, client-centered case management to families and Transitional Age Youth (TAY) residing in a 24-hour interim housing facility. This role is crucial to helping clients stabilize, access vital services, and transition into permanent housing.

Key Responsibilities Case Management & Service Planning
  • Conduct biopsychosocial assessments and develop individualized service plans (ISPs).
  • Provide ongoing support, crisis intervention, and goal-oriented case management.
  • Ensure timely and accurate DMH documentation, including treatment plans and progress notes, to maintain accurate records.
Service Coordination
  • Link families to community resources, healthcare, childcare, education, and mental health services.
  • Coordinate transportation, referrals, and advocacy with external providers.
Housing Navigation
  • Support families with housing search, applications, and lease-up processes.
  • Collaborate with housing authorities, landlords, and inspection services.
  • Provide pre- and post-move support, as well as transition planning.
Crisis Response
  • Deliver on-site crisis intervention, safety assessments, and de-escalation.
  • Partner with security and clinical teams to effectively manage emergencies.
Specialized Support
  • For TAY families:
    Offer age-appropriate services including vocational training, parenting support, and life skills development.
  • For general families:
    Provide family strengthening resources, parenting support, and referrals to mental health and child welfare services.
Team Collaboration
  • Participate in multidisciplinary meetings, case conferencing, and treatment planning.
  • Work closely with clinical supervisors, program staff, and community partners to ensure effective collaboration and support.
Qualifications
  • Bachelor’s degree in Social Work, Psychology, or related field (Master’s preferred).
  • 2+ years in homeless services, with at least 2 years in a lead or clinical setting.
  • Deep knowledge of Trauma-Informed Care, Housing First, Motivational Interviewing, and Harm Reduction.
  • Strong crisis response, documentation, and organizational skills.
  • Proficient in HMIS and Microsoft Office.
  • Bilingual (English/Spanish) strongly preferred.
Requirements
  • Valid CA Driver’s License, insurance, and reliable transportation.
  • TB test, background clearance, and CPR/First Aid certification.
  • Must qualify for UBH driver insurance.
