Position Summary
The Case Manager works under the direction and supervision of the Senior Manager of Hope Center (SMHC). The SMHC is responsible for providing case management to include, but not limited to: development of case plans, reviews, medication monitoring compliance, financial/budget mentoring, debt advocacy, employment development/referrals, assistance with application for housing, providing community resources/referrals, etc. The SMHC also participates in the continued development and implementation of program related goals.
Essential Functions
- Understands the mission, organizational structure, and policy and procedures of The Salvation Army. Works cohesively with the Senior Manager, CIS Director, Executive Director, VA Liaison, and Hope Center staff.
- Provides community-based transitional housing and supportive services to homeless veterans to help them achieve residential stability and self-determination.
- Focus on intensive, short-term case management to assist veterans in moving into permanent housing.
- Coordinate support services from the various county agencies such as District Attorney, Behavioral Wellness and others.
- Develop, implement, and review Goal-oriented planning, ensure medication adherence, assist with financial stability and deb resolution, and provide tailored community referrals.
- Assist veterans with benefit applications (e.g., Cal Fresh/Food Stamps, General Relief, Health Care, etc.).
- Enforce House Rules and program objectives. Performs urinalysis and breathalyzer tests for residents at admittance and when directed.
- Updates, organizes, and maintains resident information in Homeless Management Information System (HMIS), Internal forms, rosters/forms and case records.
- Participates as part of the Case Management Team by attending weekly meetings, preparing documentation, and providing updates on residents’ behavior and progress in the program. Provides follow up case management and interventions with residents.
- Facilitates courses/groups designed to aid in program related goals.
- Gathers program information required for reporting monthly statistics, records and reports.
- Cross trains with other Case Management Staff in-order to provide the necessary support and knowledge required in their absence.
- Assists in preparation for inspections, reviews, internal and external audits.
- Collaborate with Santa Barbara’s Housing Hospitality GPD program to enhance client care by sharing administrative responsibilities, implementing a shared client-focused approach, and coordinating supportive service referrals.
- Performs other duties as necessary for the successful operation of the program.
- Maintain accurate time keeping records that are reflective of duties performed under different grant/contracts.
- Attends staff and program related meetings, conferences, and training as needed and required. Cooperate with the Senior Manager on any other special projects as needed.

Working Conditions
Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to operate computer, fax and telephone. Ability to lift up to 25 lbs.
Minimum Qualifications
