SummaryThe Senior Case Manager is responsible for leading a collaborative effort to house the most vulnerable and chronically homeless individuals in SPAs 4 and 5. With the support of and in partnership with the FSP team, the Senior Case Manager will ensure that FSP clients have a successful process for obtaining housing and accessing other supportive services by participating in training new staff and representing the program at Agency and Community Provider Meetings.
Provide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishment.
- Conduct screening interviews, complete housing documentation, and coordinate clients' move‑in with the housing authority, property managers.
- Perform initial comprehensive case‑management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing.
- Encourage and promote a strength‑based environment to assist clients in meeting their individual goals.
- Identify appropriate permanent housing options for clients, such as subsidized housing Section 8, Shelter Plus Care, VASH, permanent supportive housing, affordable and market‑rate housing, and other opportunities.
- Assist clients with surveying the rental market for affordable (and possibly subsidized) housing, scheduling appointments, completing supportive and subsidized housing paperwork, and applying for public benefits.
- Advocate for clients with prospective landlords and the housing authority.
- Document all services provided to the Department of Mental Health and agency standards.
- Outreach to community, realtors, landlords, housing developers, and other service providers to identify new and existing opportunities and build strong relationships to better assist clients in accessing housing opportunities.
- Actively participate in staff meetings and trainings and provide coverage to lead meetings in the absence of Program Managers.
- Provide support in training new FSP staff including sharing resources, assistance with linkages, rehabilitation interventions, and skills with clients.
- Work in collaboration with FSP Leadership to meet program outcome goals in weekly team meetings.
- Serve as a liaison between the team/agency and external community partners.
- Participate in external and internal agency trainings relevant to serving target populations and present on training materials in team meetings.
- 3 years or more of case‑management experience.
- At least 1 year of experience working with adults with severe, persistent mental illness and/or in housing services.
- Experience and familiarity with LA County resources, benefits, and services to help clients meet their case‑management needs.
- Bachelor's degree in a related field, or an equivalent combination of education and experience.
- Extensive experience working with chronically homeless clients and/or homeless veterans.
- Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
- Current, valid California Driver's License with an acceptable driving record.
- Ability to work effectively with a wide range of constituencies in a culturally competent manner.
- Strong interpersonal and team‑building skills.
- Ability to work with and lead community groups and volunteers.
- Self‑starter with the ability to stay ahead of the curve and thrive in a fast‑paced work environment.
- Knowledge of public housing authority and housing subsidy preferred (Section 8, Shelter Plus Care, VASH).
- Minimum 30 wpm typing speed preferred.
- Prior advocacy or academic experience in housing and/or homeless services preferred.
