Job Summary

This Registered Nurse (RN) position requires providing skilled nursing care, case management, and hospice care to patients facing age-related frailty and serious illness. The RN will perform patient assessments, administer medications, develop treatment plans, and collaborate effectively within an interdisciplinary team. A minimum of two years of experience is required.

• Active Registered Nurse (RN) license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum two years of experience in hospital, home health, or hospice care.
• Clinical assessment, medication administration, and treatment planning skills.
• Interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast‑paced environment.
• Driver's license, car insurance, and automobile for field work.

• Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.

• On‑call duties are expected 1-2 days per week and rotating weekends.
• This is an acute care hospital setting.

Schedule will include on‑call shifts (1‑2 days/week, rotating weekends). Specific shift assignments will be determined.
