The Clinical Nurse Consultant assumes the responsibility for maintaining and sustaining for Performance Improvement (PI), system analysis, assessment, coaching, coordination and implementation of clinical systems and tools according Facility Policies and Procedure in coordination with its leaders to improve survey results and nursing home compare rating. The job promotes standards for assuring compliance with regulatory guidelines and the delivery of quality services within the long-term care units.
The position provides oversight and direct support to facilities and corporate personnel in the development, implementation, and integration of performance improvement activities and quality care as well as oversight of survey activity implementation. The position involves responsibilities in areas of quality assurance, adverse event monitoring, performance improvement and reporting, analysis, interpretation, education, evaluation and recommendations. The position personnel’s responsible for conformance to regulatory requirements, contractual obligations, and corporate policy by the facility's quality improvement program through detailed understanding and can skillfully use the techniques of system design, reengineering, project management, quality improvement, outcomes measurement, and statistical analysis.
Relationship
Collaborates with all levels of facility leadership as well as with facility consultants, physicians in assessing needs for survey management, outcomes measures of quality of care and facility star rating and provided related education. Liaison to facility leadership, interdisciplinary team, ancillary departments and facility quality management/compliance personnel as a resource in effecting performance improvement systems. Liaison to corporate personnel as a resource in providing education/training, establishing protocols, and monitoring of overall performance improvement activities in the facilities as well as assessment of quality service needs and the implementation of processes to achieve and monitor ongoing compliance and to assist the evaluation and implementation of performance improvement activities.
Qualifications- PROFESSIONAL
- Current state licensure in the which the facilities are located and supported for.
- Completed nursing program or equivalent required and graduated from an accredited school of nursing.
- Strong background and deep knowledge of the Resident Assessment Instrument (RAI)
- Proficient with the use of Point Click Care
- Progressive nursing experience in the field of long term care with demonstrated leadership and teaching abilities.
- Experience in SNF setting with knowledge of long term care regulations,
- Demonstrated skill in management principles of planning, organizing, and directing.
- Interacts with facility owners and operational management. May interact with patients, families and visitors in post-acute setting.
- Carries out other tasks as requested where hands-on intervention and participation maybe required.
- Current valid licensure to practice as a Registered Nurse or Licensed Vocational Nurse in California
- Current Basic Life Support Certification
- Minimum of 5 years nursing experience required, 2 of which has been in supervision

- Must have strong communication skills, both oral and written
- Must have knowledge and awareness of the care needs of the elderly and/or chronically ill.
- Must have knowledge and ability to teach adult learners.
- Must have basic computer skills.
- Must be able to organize, analyze, investigate and implement principles of infection control committee.
- Must be able to travel
