Overview

Actively contribute to the development of the Registered Nurse role. RNs act as a bridge between Patient’s Services and Provider care, establishing a strong working relationship with the Clinic Manager and Lead Provider  work closely with the Director of Clinic Operations, Lead Provider, and Medical Leadership, and with the Clinic Manager to improve patient flow and integration of patient programs.

Responsibilities
  • Serve as the center’s on-site clinical liaison.
  • Ensure core clinical competencies of CMAs, CHWs and care managers; train direct patient care staff to be proficient in clinical competencies.
  • Monitor performance improvement plans and collaborate in developing team priorities, patient goals, chronic disease care management, symptom-based triage and screening guided by guidelines and procedures.
  • Participate in dissemination of patient education, patient self-management teaching, coaching and support, panel management, and care coordination.
  • Be proficient in the use of the electronic health record and practice management systems; assure compliance with all agency policies and procedures.
  • Provide consultation to facilitate understanding and influence the plan of care; analyze assessment data to determine health issues and nursing diagnoses.
  • Perform symptom-based triage and screening; facilitate patient understanding of the physician’s treatment plan, including prescriptions, refills, medical supplies, referrals, and services authorization.
  • Coordinate patient lab calls with providers and track patient progress; adjust individualized care management as needed.
  • Collaborate with diverse patients and staff across cultural, socioeconomic, educational, religious, racial, ethnic, and age groups.
  • Deliver professional nursing services as defined by practice standards; may cover clinical areas such as laboratory and dispensary.
  • Ensure safety, ethics and confidentiality; participate in quality improvement programs and disaster preparedness as applicable.
  • Assist with development and adherence to policies and procedures for case management, clinical workflows, and PCMH integration.
  • Promote the center’s services and participate in community outreach; communicate with care coordinators, care teams and physicians.
  • Participate in education programs for patients with chronic disease diagnoses and support ongoing staff education and development.
  • Develop and oversee MA functions, procedures, and data-entry requirements where applicable.

Qualifications
  • Education
    • Associate in Nursing - Bachelor Degree Preferred
    • RN licenses required
    • Bilingual English/Spanish (Preferred)
  • Experience
    • Supervisory, Teaching/Education, Process Evaluation and Creativity. Community health experience preferred.
  • Licensure/Certification
    • Employees are responsible for maintaining individual certifications as required by job function or by law.
    • Verification and recertification when requested by management.
