Position Overview:

We are seeking a  Bilingual Nurse Practitioner (Vietnamese)  to join our patient-centered geriatric wellness team. This is a  non-primary care  role focused on completing  Medicare annual health assessments  and  preventative screenings  both in-clinic and through home visits. We offer full training, ongoing support, and all the tools you'll need to succeed.

Who You'll Work With:
You'll care for a  geriatric patient population , conducting  comprehensive health assessments , reviewing risk factors, and supporting continuity of care in partnership with patients’ primary care providers.

Key Responsibilities:

Conduct Medicare  Annual Wellness Visits (AWVs)  in both clinical and home settings

Perform  History & Physicals (H&Ps)  and preventative health screenings (PAD, neuropathy, diabetic retinal exams, A1c, kidney function tests)

Document using EMR and digital tools (laptop/tablet provided)

Patient Load:

7–8 patients/day in clinic

3–5 patients/day during home visit days

Travel between  Fresno & Visalia  as needed

Collaborate with PCPs to ensure patients receive follow-up care when necessary

Training & Support:

Comprehensive onboarding & training provided  — ideal for new grads

All medical equipment, laptops, and tablets are supplied

You'll work in a  supportive, team-oriented environment  that values communication and clinical excellence

✅  Requirements:

Current Nurse Practitioner license  in the State of California

Fluent in  Vietnamese  (Required)

Proficient in using computers and  EMR systems

Strong interpersonal skills; professional and flexible attitude
