Responsibilities- You will be responsible for the planning and coordination of all aspects of Post Kidney Transplant patients’ evaluation throughout the continuation of services to follow-up care.
- Serves as the primary coordinator for the interdisciplinary team, including Physicians, Nurses, Social Workers, Dieticians, Pharmacists, Financial Analysts, Case Managers, and support staff.
- Develops, coordinates, implements, and evaluates informative programs/projects for patients, personnel and the community.
- Collects/documents data to meet clinical, financial, and regulatory requirements.
- Participates in the development of guidelines, protocols, pathways and other performance improvement activities.
- Participates in research activities.
- Serves as a patient advocate throughout the transplant/surgical process.
- Represents the Medical Center at regional and national meetings/conferences.
- Education: Bachelor of Science, Nursing (BSN) required
- Licenses/Certifications: Current, unrestricted California RN License required; BLS from the American Heart Association or American Red Cross required; CCTN or CCRN Acute/Critical Care Nursing or PCCN Progressive Care Nursing Certification or other specialty certification required;
Certified Transplant Coordinator certification preferred (or willing to obtain within one year from start date) - Experience: A minimum of 3 years of acute care nursing experience required; ICU or Transplant experience preferred
Base Pay
: $58.96 - $94.34
