About the RoleThe Supportive Care Medicine (SCM) Triage Nurse provides initial triage assessment of inpatient referrals to the Supportive Care Medicine consultative service and helps assess the urgency of new consults. The role works with the interdisciplinary team to manage consultation workflow and serves as a primary resource for referring clinicians and the patient’s clinical team. The triage nurse assists with transitions from inpatient to outpatient care, provides initial and follow‑up visits for on‑service patients, and supports the SCM team with communication, decision‑making, and patient safety.
Job Duties and Responsibilities- Conduct initial and follow‑up triage nursing assessments for SCM
- Identify priorities for patient care management
- Enact problem‑solving and critical‑thinking skills
- Report concerns or urgent messages to the interdisciplinary team
- Collaborate in providing patient care
- Develop and maintain therapeutic relationships
- Promote patient safety
- Facilitate smooth transitions between care environments
- Provide effective patient/family/staff teaching
- Provide educational in‑services to peers and referral sources on the topic of Supportive Care Medicine
- Utilize resources effectively and efficiently
- Implement transitions of care between inpatient SCM and continued outpatient follow‑up
- Adhere to scope of practice/Cedars‑Sinai standards and facilitate continuous improvement of processes/services
- Maintain and enhance professional skills
Education: Graduation from an accredited school of nursing required;
Bachelor of Science in Nursing degree preferred.
License/
Certifications:
Valid CA RN license required;
Certified Case Management (CCM) RN preferred.
Experience: 3 years of acute nursing experience required; experience in palliative care, oncology or critical care and/or project management preferred.
