Overview
Health Advocates Network is currently seeking a Registered Nurse to work at a facility in Los Angeles, California.
CompensationPay Rate: $68 / hour.
W2.
40-hours per week. Days 13-weeks. Nursing Specialty: RN-Interim Housing.
Qualifications- Active Registered Nurse license
- AHA BLS
- MAB or CPI
- LA City Fire and Life Safety card (must be LA City)
- Proof of full COVID and seasonal Flu Vaccinations
- MAB (Management of Assaultive Behavior)
- Fire Card
- Must have recent experience within the last 3 years in the area they are staffing
Provides critical clinical subject matters expertise and support to contracted IH providers addressing and supporting the medical and behavioral health needs of participants at IH recuperative care and stabilization sites. IH staff also support with the participants Transition of Care (TOC) post-hospital discharge. This is done through completion and review of participants referrals and assessments care plan development and coordination, client / facility in person visits, ongoing evaluation and monitoring of participants cases in collaboration with an interdisciplinary team.
RN will assess support service and provide training and technical assistance to IH providers engaged in long term care of their clients.
