SUMMARY

The Registered Nurse (RN) plays a vital role in delivering high-quality, patient-centered care at Gracelight Community Health. This position is responsible for providing clinical oversight, care coordination, and patient education while ensuring compliance with evidence-based guidelines and regulatory requirements. The RN collaborates with multidisciplinary teams to support chronic disease management, triage, and care planning, contributing to the organization’s mission as a Patient-Centered Medical Home (PCMH).

Additionally, the RN supports staff training, quality improvement initiatives, and adherence to best practices in clinical operations.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
  • Supports and implements the organization’s vision, mission, and values.
  • Determines priorities to complete daily workload to ensure that all responsibilities are carried out in a timely manner.
  • Performs symptom-based triage and screening in accordance with clinical guidelines and policies.
  • Provides professional nursing assessments, prioritizes treatments based on urgency, and initiates appropriate care.
  • Administers medications and vaccines, ensuring compliance with the Five Rights of Medication Administration.
  • Oversees medication reconciliation, patient education, and adherence to prescribed treatments.
  • Supports the management of acute and chronic disease states through evidence-based interventions.
  • Facilitates care coordination with health plans, hospitals, and community partners to ensure continuity of care.
  • Monitors and ensures compliance with CMS, NCQA, DHCS, and other regulatory standards.
  • Conducts chart reviews, documentation audits, and quality control measures to maintain high standards of care.
  • Maintains department compliance with HIPAA regulations to protect patient health information.
  • Participates in disaster preparedness programs and other compliance initiatives as required.
  • Acts as a liaison between patients, providers, and external agencies to ensure seamless care transitions.
  • Provides patient education, self-management support, and chronic disease coaching.
  • Assists in the implementation of panel management, case management, and population health strategies.
  • Encourages patient adherence to treatment plans, including medication regimens and preventive screenings.
  • Supervises Medical Assistants (MAs) and Community Health Workers (CHWs) to ensure clinical competency.
  • Provides training, mentorship, and performance evaluations for direct patient care staff.
  • Develops and maintains policies, procedures, and training programs to enhance clinical effectiveness.
  • Supports the onboarding and orientation of new clinical staff.
  • Utilizes Electronic Health Records (EHR) and practice management systems for accurate documentation.
  • Manages medical inventory, including prescription drugs, vaccines, and controlled substances.
  • Recommends process improvement measures to enhance clinical operations and patient outcomes.
  • Participates in committees, work groups, and quality improvement initiatives as assigned.
  • Demonstrates flexibility to change and to evolving healthcare needs, organizational changes, and advancements in the field.
QUALIFICATIONS, SKILLS & ABILITIES
  • Registered Nurse (RN) licensed in California.
  • Bachelor's degree in nursing (BSN) preferred.
  • Experience in care management, preferably in a primary care setting.
  • Strong clinical and care coordination skills.
  • Excellent written and verbal communication skills, utilizing tact, active listening, empathy, and compassion. Able to manage conflict in a direct and professional manner.
  • Ability to work effectively in a team-based care environment.
  • Proficiency in electronic health record systems and documentation.
  • Experience working with diverse populations from various social, cultural, educational, and economic backgrounds.
  • Valid driver’s license with current automobile insurance.
  • Current CPR certification required.
  • Ability to organize and prioritize work tasks.
  • Proficiency with computer applications such as Microsoft Excel, Word, and PowerPoint; EPIC.
  • Bilingual (Spanish, preferred).
  • Strong problem-solving and critical thinking skills.
  • High level of cultural competence and sensitivity.
  • Flexibility and adaptability in dynamic work environments.
  • Strong organizational skills and attention to detail.
  • Regular on-site presence required.
