Overview
As an integral part of the interdisciplinary team, the Registered Nurse utilizes the nursing process to provide therapeutic care to patients. The Registered Nurse is accountable for the coordination of patient care and education to the patient and family in accordance with the standards, policies, and practices in a compassionate, supportive, and therapeutic environment.
Essential Duties- Follows policies and procedures; demonstrates ability to access online manuals.
- Transfers learned scientific knowledge into practical application in applying the Nursing Process.
- Assesses appropriately on patient admission, at each shift, and with change of condition. Completes patient acuity by 0200/1400 daily.
- Interprets assessment information and formulates Nursing Diagnoses.
- Develops Plans of Care with individualized interventions. Documents expected/projected patient outcomes. Reviews, revises, and updates Plans of Care daily based on evaluation and interpretation of patient outcomes.
- Administers medication and IV therapy appropriately at prescribed intervals. Completes patient care procedures correctly per policy. Delegates tasks based on others’ scope of practice and competency. Acts as patient advocate. Manages restraint use per policy, exploring non-restraint measures first. Ensures MD orders and assessments per policy. Ensures patient monitoring per policy. Completes Restraint Audit Form each shift.
- Documents complete and correct information on all flow sheets and chart forms (Admission Assessment, Pain scale assessment and reassessment, Daily flow sheet, Restraint, Diabetes flow sheet, Educational flow sheet, Care Plan, Risk Assessment & Reassessment, MAR, and related forms). Transcribes and signs off orders prior to the end of the shift. Completes 12h/24h chart checks.
- Provides patient/family education as needed (Room/unit orientation, Plan of Care). Identifies knowledge deficits related to diagnoses and care. Uses MICROMEDEX information for patients and families.
- Collaborates with Case Manager and/or physician regarding discharge planning. Ensures all transfer paperwork is completed. Ensures belongings are sent with patient.
- Utilizes patient care equipment safely and according to manufacturer guidelines (IV pump, Feeding pump, PCA pump, Epidural pump, Sequential compression device, Wound VAC, Heating pad/pump).
- Communicates effectively with all members of the healthcare team including patient and family (shift-to-shift reports, physician rounds, patient/family interactions, hand-off reports, emails, and protocol adherence).
- Manages time effectively and prioritizes responsibilities. Takes breaks as needed and completes shifts on time.
- Adheres to regulatory agency requirements:
National Patient Safety Goals, patient identifiers, universal protocol, medication reconciliation, clinical alarms, hazard waste management, HIPAA, OSHA, JCAHO readiness, and critical values. - Observes and documents safety practices: correct body mechanics, electrical safety, infection control, hand washing.
- Demonstrates capability to manage medical-surgical patients (6N), including THA, TKA, spinal surgery, isolation precautions, and special care for hepatic encephalopathy and transplant patients.
- Demonstrates ability to manage ICU/PCU patients with advanced therapeutic modalities (hemodynamic monitoring, CRRT, intracranial pressure monitoring, VADs, etc.).
- Manages step-down/telemetry, including high-acuity patients and post-transplant cases; uses ventilators, arterial/CVP lines, VADs, lumbar drains, etc.
- Initiates and terminates dialysis treatments per policy; performs pre-treatment safety checks and coordinates with physicians for orders.
- Engages psychiatric patients in therapeutic interactions, maintains therapeutic milieu, and participates in treatment planning; monitors safety and adheres to legal status requirements; administers psychotropic medications with proper consent and documentation.
- Utilizes de-escalation techniques and applies behavioral restraints only as appropriate; ensures physician assessment within required time frames and completes debriefing when applicable.
