Shift: 40 hours per week, Monday-Friday, with occasional on-call coverage. Schedule will vary

Salary: $49 - $61k;
Eligible for performance cash bonuses

Position Information: In this role, you will be responsible for the management and delivery of direct nursing care to PACE (Program of All-inclusive Care for the Elderly) participants in a variety of settings including, but not limited to, the clinic, care center, home, virtually, SNF and beyond. This individual will serve as a critical member of the Interdisciplinary Team (IDT) and work collaboratively to complete assessments and drive forward participant care plans.

Qualifications:

• Associate degree or higher
• Wound Care (4+ years)
• RN (4+ years)
• 3+ years of experience in ANY of the following:
  • Geriatric Care
  • Senior Care
  • Geriatric/LTC
• Valid American Heart Association Basic Life Support (AHA BLS)
• Valid Registered Nurse (RN) in the state of California (CA)
