Compensation: $95,000 to $103,000 (DOE) plus 15% annual bonus potential.
This is a full‑time, on‑site position located in Woodland Hills, California.
Principle Duties- Provide nursing evaluation of residents, coordinate additional care, and communicate with medical personnel and family members, with follow‑up after emergency events.
- Coordinate residents’ return, including transportation, medications and a light meal; update plans of care.
- Provide individual and group educational opportunities for residents and staff on disease prevention and management in the holistic spirit of the Lifestyle and Health Services Wellness Program.
- Facilitate collaboration with other leaders to identify and recommend additional services or a physical move within the continuum of care for residents whose needs change.
- Anticipate and coordinate with residents and families when significant changes in condition necessitate additional support services or higher‑level care transfer.
- Offer emotional support to residents and families dealing with the aging process and illnesses.
- Oversee medication technicians and LVN staff to ensure standards for service delivery to residents; manage clinic services including pre‑admission health screening.
- Support chronic condition management, coordinate clinical visits by other professionals, and assist with medications and ADLs.
- Interview, select, train, supervise, and manage performance of all residential health services staff.
- Ensure all staff training is completed and documentation maintained.
- Perform other duties as assigned by supervisor.
- Current California Licensed Vocational Nurse License in good standing with the LVN Board of Nursing.
- RCFE preferred.
- CPR, First Aid, and AED required.
- Five years’ experience working with seniors in long‑term care, home health or another health‑care setting is preferable.
- Five years of organizational or management experience; supervising others is preferable.
- Passion for working with the elderly, strong clinical skills, and strong customer‑service, leadership, and interpersonal skills.
