Responsibilities

A Patient Care Manager contributes to AHF by utilizing experience, skills, education, and a thorough understanding of the Ryan White Care Act Program within their area. You will work with our providers and Healthcare Center team so that every Ryan White patient receives the support they require and access to available resources.

Responsibilities include:

  • Providing education and assistance so that eligibility for the Ryan White program is understood.
  • Meeting with patients in the Healthcare Center to complete health risk/needs assessments and develop integrated care plans.
  • Providing ongoing education to patients to understand their diagnoses and medications.
  • Conducting home visits as clinically appropriate to complete assessments or follow up on patient needs and ensure equitable access for transportation or mobility barriers.
  • Understanding mental health, addiction challenges, and basic needs (housing, food, clothing) in Ryan White contracts; developing relationships with programs and agencies serving those needs; facilitating referrals and support for patients lacking these services.
  • Maintaining an active network of community resources to support housing stability, food access, transportation, behavioral health, legal aid, and other social determinants of health.
  • Conducting ongoing retention calls to ensure continuity of care; coordinating with MCC teams and Retention Outreach Specialist to implement interventions aligned with each client’s stage of change and acuity.
  • Enhancing the organization’s reputation through case management excellence and collaboration with providers to promote viral suppression and better health outcomes.
  • Coordinating with primary care providers for case conferences; leading discussions on barriers to care (mental health, addiction, etc.); developing integrated care plans with RNMCM and providers; monitoring needs over time.
  • Conducting monthly peer reviews to assess documentation, care planning, and service delivery to meet AHF MCC program standards.
  • Ensuring eligibility verification for the ACA and initiating enrollment for qualified individuals.
Qualifications
  • Master’s degree in Social Work, Counseling, Psychology, Marriage and Family Counseling, or Human Services.
  • A minimum of 3 years of Case Management experience.
  • Knowledge of Ryan White Services is preferable.
  • Competence in Cultural and Linguistic Diversity.
Licenses And Certifications
  • Master’s degree in one of the listed disciplines (see Qualifications).
