Horizon San Gabriel Valley is seeking a compassionate and organized Admissions Coordinator to play a vital role in our Recuperative Care program. This position is ideal for someone who enjoys working with people, coordinating details, and ensuring a smooth, supportive admissions experience for patients transitioning into care.
Position OverviewThe Admissions Coordinator serves as a key liaison between prospective patients and referring organizations. You will guide individuals through the admissions process, ensure all required documentation is complete, and provide hands-on support to facilitate a positive and seamless entry into Recuperative Care.
Key ResponsibilitiesAct as the primary point of contact for prospective patients and referral partners
Coordinate and manage the admissions process from referral to acceptance
Ensure all required documentation is completed accurately and on time
Communicate clearly with hospitals, social workers, and referral organizations
Provide compassionate guidance and support to patients during intake
Maintain organized and confidential patient records
Collaborate with internal teams to ensure smooth admissions and transitions
Prior experience in admissions, healthcare coordination, case management, or related role preferred
Strong communication and interpersonal skills
Excellent organizational and documentation skills
Ability to work independently and manage multiple priorities
Compassionate, professional, and patient-focused approach
Experience in recuperative care, healthcare, or social services a plus
