• Monday - Friday 8:30am - 5:30pm with lunch break
• 15 patients per day with longer visits for continuity of care
• Clinic setting
• Geriatrics training and PACE program experience required
• Patient population 55 years and up
• Hospital privileges required
• BLS certification required
