Position: Case Manager (Family Home Specialist)

Overview

California Institute of Health & Social Services (CIHSS) is seeking an experienced Case Manager (Family Home Specialist) to oversee compliance and quality care for neurodivergent adults in family home settings. This is a compliance-focused role requiring significant fieldwork, documentation expertise, and regulatory knowledge.

Responsibilities
  • Conduct bi-monthly face-to-face home visits to assess health, safety, and HCBS compliance.
  • Develop and submit individualized service plans (ISPs), monthly reports, and clinical documentation.
  • Coordinate care and resources for assigned clients.
  • Provide crisis-response support and initiate SIR reporting per Title 17 protocols.
  • Ensure compliance with Title 17 regulations and HCBS-Final Rule standards.
  • Maintain accurate data entry in the Therap platform.
Qualifications
  • Bachelor’s degree in psychology, social work, health services, or a related field.
  • 2-4 years of experience in developmental services case management.
  • Applied knowledge of Title 17 guidelines and HCBS-Final Rule compliance.
  • Valid California driver’s license, clean driving record, and a reliable personal vehicle for frequent field visits.
  • Strong written and verbal communication skills.
  • First Aid/CPR certification (attainable within 10 days of hire).

Pre-Employment Requirements
