Registered Nurse - Case Manager

Responsibilities

• Perform care coordination and discharge planning for patients.
• Organize and participate in family meetings.
• Participate in the development of treatment plans.
• Utilize computerized documentation systems and Microsoft applications proficiently.
• Coordinate with interdisciplinary support such as Social Services and Transportation.
• Arrange DME, SNF, and ambulance transport independently.
• Adhere to admission criteria and assess patients effectively.
• Work with HMOs, IPAs, and understand the protective services process.

• Case Management/Utilization Review
• Care Coordination
• Discharge Planning
• Plan of Care Regulatory
• Proficiency in Inter Qual Criteria*
• Knowledge of DRG (Diagnosis Related Groups)*
• Familiarity with CMS:
  Centers for Medicare and Medicaid Services Work Settings

• 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge planning.
• Experience in an acute hospital setting is required.
• Travel experience required, first-time travelers accepted.
