At Healthcare in Action, we serve patients experiencing homelessness through an innovative “street medicine” approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.
** This is a limited-term position, with an anticipated term of one year, depending on performance and funding.

Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks.

The Lead Peer Navigator (Liaison) will act as a liaison between HIA and City of Los Angeles - Council District 4.The Lead Peer Navigator (Liaison) will collaborate closely with Council District 4 and HIA to coordinate and enhance service delivery to support individuals experiencing homelessness across the neighborhoods within Council District 4.  The Lead Peer Navigator (Liaison) will work directly with City staff and in the City offices or out in the field, depending on assigned duties and responsibilities.  

This position is dependent on funding from Council District 4, and the position, duties and responsibilities are subject to change.

We are seeking thoughtful, hard-working, committed and compassionate team members to work in a collaborative, fast-paced environment. Public engagement, field outreach, and government experience are valued, along with public speaking and consensus-building experience. The Lead Peer Navigator (Liaison) may need to present at public meetings, and/or build collaborative work groups with key stakeholders. Most important is that the Homeless Deputy must be self-starting, organized, energetic, and dedicated to bringing about positive change in the lives of unhoused and housed constituents.

* Responsibilities include:

*** Collaborate with Council District 4 leadership, staff, and service providers to align and streamline homelessness strategies and encampment interventions.
* Conduct daily outreach, encampment assessments, and wellness checks to support individuals experiencing homelessness across the district.
* Respond to inquiries, attend community meetings, and deliver updates on homelessness-related efforts and policies.
* Present at meetings, lead work groups, and engage key stakeholders.
* Maintain accurate service records, analyze trends, and prepare regular reports to inform strategy and public communication.
* Coordinate sanitation responses and oversee events like Neighborhood Support Days to ensure services are delivered with dignity and impact.
* Other duties as assigned.
** Qualifications and experience:
*** G.E.D./High School, required.
* Bachelor’s degree preferred, 1-2 years of work experience as defined below.
* Work Experience to include direct work experience, lived experience, training or volunteer work with patients experiencing homelessness, as well as those with active mental health and substance use conditions.
* Other demonstrated relevant experience in similar community settings, for example, working with other disadvantaged groups, and/or outreach will be considered.
* A comparable combination of education/experience and/or training will be considered equivalent to the education listed above.
* Compassion, commitment to, and enthusiasm for addressing the concerns and needs of Council District 4 residents, including both unhoused and housed residents
* Ability to work in a dynamic, outdoors environment.
* Ability to work independently as well as part of a team.
* Ability to organize effectively and prioritize multiple and competing tasks.
