Join our Street Medicine team as the vital link between street‑based outreach and the healthcare system. This role splits between office‑based administrative work (70%) and field clinical support (30%), focusing on care coordination and navigation for our most vulnerable patients through our Enhanced Care Management (ECM) program.
What you’ll do Administrative & Care Coordination (70%)- Manage referrals to specialists: initiate, schedule, track, and ensure follow‑up
- Secure prior authorizations for medications, imaging, and specialist visits
- Verify insurance eligibility (Medi‑Cal, Medicare) and assist with enrollment issues
- Maintain medical supply inventory for field outreach
- Document patient encounters, referrals, and authorizations in EHR system
- Support ECM program through patient tracking, scheduling, and care coordination
- Serve as office point of contact for patients, pharmacies, and community partners
- Other duties as assigned
- Accompany provider (MD/DO/NP/PA) on street outreach to encampments, parks, and shelters
- Build trusting relationships with patients and perform field intake
- Take vital signs, assist with clinical procedures, wound care, and specimen collection
- Prepare and administer medications/vaccinations under supervision
- Other duties as assigned
- Medical Assistant certification from accredited program
- Current BLS certification
- High School Diploma or equivalent
- Valid California Driver’s License with clean record
- Strong computer and organizational skills
- Exceptional empathy, compassion, and communication abilities
- Bilingual (English/Spanish)
- 1+ years clinical administrative experience with prior authorizations/referrals
- Additional certifications (EMT, CNA, Phlebotomy)

Work may include both sedentary office duties and active engagement in the field, requiring walking and standing for extended periods.
Salary range: $58,240 USD - $58,240 USD
