Community Health Worker

Akido is rebuilding the healthcare experience from the ground up. Through early interventions designed around social determinants of health (SDoH), world‑class care focused on chronic disease, and human‑centered technology, we believe we can build a healthcare model that allows all patients to live their fullest lives. We are a fast‑growing, impact‑focused, Y Combinator company created out of the University of Southern California’s D‑Health Lab with the idea that empowering government, healthcare, and nonprofit services with population‑based data could help usher in a new era of preventive public health.

Today we are building a full stack medical network that leverages our predictive capabilities to provide a frictionless experience for both our patients and care providers.

Community Health Worker (CHW) will support members with complex medical and social needs, managing their own health and wellbeing. The CHW will assist in motivating behavioural changes in patients to improve health outcomes through education, peer support, and the relaying of shared experiences.

• Manage a caseload of patients, completing assessment forms, developing care plans with patients and clinical teams, and carrying out activities according to the care plan.
• Build rapport with patients to increase the likelihood of positive behavioural changes.
• Coach patients to minimize risks associated with common health conditions and behaviours.
• Acompañar a miembros a citas médicas y de servicios sociales.
• Connect members to appropriate programs to address barriers to care and enhance compliance.
• Link members to local, county and state services, follow up and serve as a member advocate.
• Introduce systems to promote self‑management and self‑efficacy.
• Document information from every encounter in designated information systems.
• Outreach and engage with eligible patients to enroll them into the Akido ECM Program.
• Other duties as assigned.

• High School Diploma or equivalent.
• Bilingual in English and Spanish strongly preferred.
• 2+ years of experience working with CalAIM populations of focus (people experiencing homelessness, adults with SMI/SUD, adults transitioning from incarceration, adults with complex medical needs).
• Ability to work in a dynamic, outdoor environment.
• Ability to work independently as well as part of a team.
• Ability to prioritise multiple and competing tasks.
• Excellent oral communication skills and strong interpersonal skills.
• Ability to communicate effectively, including articulating one’s own relevant personal experiences.
• Ability to use computers to document information into case‑management software.
• Travel 50‑75% – must have a valid driver’s licence, automobile insurance and reliable transportation.

$28 per hour
