Overview

The Community Health Worker (CHW) offers preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health for the participants and community served. The CHW serves as a navigator to link healthcare and social services to the community through a culturally responsive lens to improve quality of care.

Essential Duties
  • Conducts outreach activities to identify community members who may benefit from health education and support services. Provides culturally appropriate health education to individuals and families in the community, including topics such as chronic disease management, healthy lifestyle choices, and preventative health measures.
  • Advocates for community members  needs and connect them with community resources and services that address the social determinants of health, such as housing, food, and transportation.
  • Supports participants to enroll or maintain enrollment in Medi-Cal or other assistance programs that are related to improving their health if such navigation services are provided pursuant to a plan of care.
  • Outreaches to potential members from a call list to inform them of the program and its benefits and enroll them in services. Outreach includes phone calls, in-person visits, letters, emails and text messages.
  • Attends assigned community events and meetings on behalf of CII to represent the organization and support program visibility and enrollment.
  • Serves as a liaison between participants, healthcare team, and other community resources.
  • Establishes and maintains rapport and effective working relationships with children and families and works effectively within CII and the community at large.
  • Initiates and maintains communication flow between community groups and organizations.
  • Accompanies program participants to office visits, as needed and according to applicable guidelines.
  • Develops and maintains effective communications with appropriate stakeholders.
  • Collaborates with multi-disciplinary teams, participates in meetings with the CII Community Engagement Team, and required meetings/trainings by funder as assigned.
  • Maintains complete and accurate documentation ensuring compliance of service standards and policies as stipulated by contract.
  • Other duties and special projects as assigned.

Qualifications EDUCATION & EXPERIENCE
  • High school diploma or general education degree (GED) required.
  • Bachelor's degree in any field preferred.
  • Community Health Worker Certification Required and/or ability to complete CHW Certificate Program within 6 months of employment.
  • Lived experience in communities for this program and/or one year of community-based direct service experience.
Other Qualifications
  • Maintain annual requirement of 6 hours of continuing education courses
  • Ability to work well with others, gain consensus, and work towards a common goal.
  • Ability to work collaboratively with healthcare providers, social service agencies, and community-based organizations.
  • Ability to identify and research community resources, and initiate/develop/cultivate new professional relationships with potential members and program partners
  • Ability to work with minimal supervision.
  • Strong interpersonal skills and critical thinking abilities
  • Must have access to reliable transportation.
  • Establish and maintain effective working relationships with those contacted in the performance of required duties
  • Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
  • Ability to handle confidential information.
  • Sensitivity to service population’s cultural and socioeconomic characteristics.
  • Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
  • Excellent verbal and written communication skills.
  • Punctual and dependable attendance.
