Our UCLA Health Neuropsychiatric Hospital is seeking to hire a Licensed Clinical Social Worker to help support our Adult PHP Program. This specialized program provides comprehensive, high-level mental health care to adults experiencing severe mental illness. Our multidisciplinary team delivers care through group therapy, individual psychotherapy, and psychiatric medication evaluation in a supportive, evidence-based setting.
Responsibilities- Formulate individualized treatment plans based on comprehensive patient assessments
- Provide individual and/or group psychotherapy
- Document and enter patient information accurately and timely into medical record
- Evaluate patient needs and refer to additional or alternative levels of care as appropriate
- Develop and implement social work interventions aligned with treatment goals
- Discuss charge plans and ongoing treatment recommendations with patients and the care team
- Identify collateral contacts and gather relevant information from external sources
- Facilitate meetings with patients’ support system
- Licensed Clinical Social Worker (LCSW) or eligible in California
- Experience in adult psychiatric or PHP settings preferred
- Strong assessment, documentation, and multidisciplinary collaboration skills
- Ability to work in a fast-paced, evidence-based environment
