Wellness Equity Alliance (WEA) is a novel national public health organization comprised of a multidisciplinary team of population and public health experts with backgrounds in infectious disease, public health, emergency medicine, primary care, cardiology, pediatrics, psychiatry, community health work (CHW), nursing and advanced practice pharmacy. We work nearly exclusively with underrepresented communities, fundamentally addressing health‑care disparities and the social determinants of health (SDoH) that have been amplified during the COVID‑19 pandemic, prioritizing the following:

- LGBTQIA+ communities
- Justice‑impacted communities
- People experiencing homelessness
- Indigenous communities
- Immigrant communities

The Case Manager plays a vital role in delivering comprehensive, compassionate, and accessible case management services to individuals and families across the Los Angeles and Inland Empire regions. This position focuses on supporting clients through critical transitions—whether from incarceration, homelessness, or other life challenges—by addressing their mental, emotional, and social well‑being through individualized assessments, counseling, treatment planning, and linkage to community resources.
Key Responsibilities- Lead and supervise CHWs and CPSWs, providing guidance, mentorship, and performance support to ensure high‑quality service delivery.
- Oversee daily field operations of the Mobile Health Clinic, including logistics, deployment scheduling, and supply management.
- Collaborate closely with medical, behavioral health, and administrative leadership to ensure seamless communication, coordination, and integrated care delivery.
- Contribute to program evaluation and quality improvement efforts, identifying opportunities to enhance service accessibility and patient outcomes.
- Support team development by promoting a collaborative, strengths‑based, and culturally responsive approach to care.
- Conduct case management, assessments, and follow‑ups for individuals with health and social service needs.
- Coordinate care across systems—linking patients to medical, health, and social support resources within their communities.
- Promote patient engagement and empowerment through education, advocacy, and supportive counseling.
- Uphold confidentiality and compliance with all federal, state, and organizational regulations.
- Provide peer support services, drawing on personal experiences to build rapport and trust.
- Advocate for health needs by assisting them in effectively communicating with healthcare providers or social service agencies.
- Offer emotional support, mentorship, and guidance to patients navigating health challenges.
- Implement programs that promote, maintain, and improve individual and overall community health within the school setting.
- Assist patients in setting and achieving personal goals, promoting resilience and self‑advocacy.
- Facilitate peer support groups and activities to encourage positive social connections and mutual support.
- Use community health needs assessments to identify areas to expand clinical services and support.
- Collaborate with CHWs and other team members to provide comprehensive and holistic care.
- Strong managerial skills with experience leading and managing field operations.
- Experience in community health work, or related fields.
- Ability to build and maintain trusting relationships with patients.
- Excellent communication and interpersonal skills.
- Knowledge of community resources and ability to assist patients in accessing services.
- Understanding of data for program evaluation and performance improvement.
- Commitment to delivering equitable healthcare and addressing social determinants of health (SDoH).
- Certification as a Community Health Worker (CHW) and/or Certified Peer Support Worker (CPSW).
