Position SummaryA Community Health Worker (CHW) contributes to AHF by blending the mission with care coordination. The CHW provides preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health. A CHW enhances the reputation of AHF by impacting improved member health outcomes and health equity which leads to longer, healthier, and happier lives.
The CHW assists members in their homes, community, or clinic setting, provides support of adherence and compliance to care, and helps members identify socio-economic issues that affect their overall health. You will be a passionate advocate for our top initiatives. The CHW will work as a part of a close-knit team to address issues that include, but are not limited to, the control and prevention of chronic conditions or infectious diseases;
mental health conditions and substance use disorders; need for preventive services, perinatal health conditions; sexual and reproductive health; environmental and climate-sensitive health issues; child health and development; oral health; aging; injury; domestic violence; and violence prevention.
- Provide preventive health services and care coordination to prevent disease and promote health.
- Assist members in home, community, or clinic settings with adherence to care and identifying socio-economic barriers.
- Work with a team to address chronic conditions, infectious diseases, mental health, substance use disorders, perinatal health, sexual and reproductive health, and other health issues.

Bilingual Spanish preferred.
