Position: Occupational Therapist (OT)

Overview

seeking a Full-Time and PRN and Part-Time Occupational Therapist to join our in-house therapy team! at Astoria Healthcare Center, a beautiful SNF in Sylmar, CA. We are a Skilled Nursing Facility and Rehabilitation located in Sylmar, CA. This position offers an exciting opportunity to be part of a team of dedicated health care professionals that pride themselves in providing quality patient care.

Our Rehab Department has an excellent reputation in the community for providing rehabilitation services.

From intensive short term rehabilitation therapy to longer-term restorative care, our Nursing and Rehabilitation Center provide a full range of services to treat each of our residents.

Duties

Duties Include:

  • Evaluate patients in timely manner following physician referral
  • Develop effective treatment plans and provide education for client, family and caregivers as subject matter expert
  • Communicate with supervisor and other health team members regarding patient progress, problems and plans
  • Participate in inservice training program for other staff in the facility
  • Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
  • Provide accurate and timely billing records and supporting clinical documentation
  • Instruct patient s families or nursing staff in maintenance programs
  • Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL s
  • Assure resident abuse or neglect does not occur
  • Observe confidentiality and safeguards for all patient information
  • Complies with federal, state, and local legal and professional requirements
