Job Description Summary

The Clinical Pharmacist – PT, PD, CA is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies and regulatory requirements. This entry‑level role coordinates patient services with nursing, monitors progress toward goals, and educates patients, caregivers and staff regarding medication use.

Job Responsibilities (listed in order of importance and/or time spent)
  • Determines suitability of individual patients for home care. Collects and organizes all patient‑specific information needed to determine suitability for home care, assessing patients' suitability for home care in accordance with Option Care policies, medical condition and prescribed medication therapy, and prognosis with clearly defined outcome goals.
  • Designs, recommends, monitors and evaluates patient‑specific pharmacy care plans for home care patients. When appropriate, makes suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters, collecting and organizing patient‑specific information to prevent, detect and resolve medication‑related problems and make appropriate medication therapy recommendations. Identifies appropriate monitoring parameters and completes nutritional assessment of patients with appropriate recommendations.
  • Modifies the pharmacy care plans for home care patients based on evaluation of monitoring data and other pertinent patient‑specific information; competent in use of pharmacokinetic principles and formulas, and in assessing patient laboratory values.
  • Provides medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques and counseling methods.
  • Ensures continuity of pharmaceutical care to and from the home and other patient‑care settings. Uses a systematic procedure to communicate pertinent patient information. Prepares and dispenses medications following Option Care policies, accurately calibrates equipment, and ensures medications are appropriately concentrated, stable and stored. Complies with state/federal laws, USP standards and DEA controlled substance regulations.
  • Manages the use, maintenance, and troubleshooting of medication administration equipment and medication‑related equipment used in the management of home care patients. Participates in the process for assessing, managing, and reporting medication‑related incidents and equipment errors, completing QAR documents and monitoring inventory discrepancies.
  • Provides concise, applicable, and timely responses to drug information requests from health care providers and home care patients, formulating responses based on literature analysis. Attends educational seminars or other CEU opportunities to promote professional competency and comply with state mandated CEU requirements.
  • Manages the use of investigational drug products according to established regulations and Option Care policies and procedures. Utilizes pharmacy support personnel effectively, prioritizing workload and organizing workflow to ensure accuracy of pharmacy support personnel work.
  • In all activities, complies with accreditation, legal, regulatory and safety requirements.
  • Maintains confidentiality of patient and proprietary information and observes legal and ethical guidelines for safeguarding confidentiality.

Basic Education And/or Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license as granted by the State Board of Pharmacy.
