- Provide comprehensive medical care by interviewing, examining, diagnosing, and treating patients.
- Deliver patient-centered primary care with an emphasis on wellness, prevention, and early detection.
- Educate patients and families on health conditions, treatment plans, and preventive care strategies.
- Coordinate specialty care based on clinical assessment and patient needs.
- Lead a team of providers and clinical staff in the management of a complex senior population to achieve high quality outcomes and guideline-based care.
- Address goals of care and end of life issues with patients and their families or caregivers in a collaborative and respectful manner
- Ensure accurate and timely medical documentation in the electronic medical record (EMR) that clearly captures and addresses all patient chronic and acute conditions
- MD or DO from an accredited medical school.
- Current, unrestricted medical license in the applicable state(s).
- Board certified or board eligible in Internal Medicine or Family Practice (if board eligible, must become board certified within 12 months).
- Active DEA license.
- Commitment to quality care and health equity.
- Passion for mentorship and team collaboration.
- Satisfactory completion of a Tuberculosis (TB) test.
