** Compensation:** $60,000 – $67,000 annually +
* Productivity Bonus* (Earn up to an additional $12,000/year!)

**** About the Opportunity
** Stepping Stone Behavioral Health is seeking dedicated and compassionate
** Licensed Social Workers (LSW/LISW), Licensed Professional Counselors (LPC/LPCC), or Licensed Chemical Dependency Counselors (LCDC II, LCDC III, LICDC)
** to join our
** outpatient behavioral health team**.  This
** mobile clinician role
** provides mental health and substance use counseling
** directly inside skilled nursing and long-term care facilities**, offering meaningful support to residents coping with emotional, behavioral, and substance-related challenges.
** What You’ll Do
*** Conduct
** bio-psychosocial assessments
** and develop individualized treatment plans.
* Provide
** individual and group counseling**, teaching coping and recovery skills.
* Identify resident needs, set measurable goals, and guide them toward emotional stability.
* Participate in
** treatment team meetings
** and collaborate with facility staff and community partners.
* Maintain timely and accurate
** clinical documentation
** in EMR systems.
* Assist with
** telehealth sessions
** and submit required daily reports.
* Facilitate safe discharge planning and coordinate aftercare services.
* Uphold confidentiality, professionalism, and compliance with facility and state regulations.
** Requirements
* ** Must be
** 21 years or older
** with a
** valid driver’s license**.
* Active
** Ohio license**: LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.
* ** Ohio Medicaid Billing Number
** and
** NPI Number
** required (or ability to obtain).
* Minimum of an
** Associate’s Degree** (Bachelor’s or Master’s preferred).
* Strong interpersonal, organizational, and documentation skills.
