Concho Hearts Hospice is dedicated to providing compassionate care wherever you call home. Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers and Doctors provide symptom management, patient and family support while coping with an end of life illness. Primary function is to provide spiritual care in the home to terminally ill patients and their families. To provide an assessment of the patient’s and family’s spiritual needs and to provide counseling to meet those needs. Makes reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient’s spiritual needs. Participates as a core member of the Interdisciplinary Group.

  Per state guidelines, if any.
• Experience: One (1) year experience as a Chaplain in a health care institution preferred.
• Skills: Working knowledge of community spiritual and other resources. Good interpersonal skills.
• Transportation: Reliable transportation. Current auto liability insurance and drivers license if personal vehicle is used for work.

• Works in patient homes and other healthcare facilities under various conditions.
• Occasional exposure to infectious diseases; ability to travel locally; may have some exposure to unpleasant weather.

• Prolonged standing, walking and sitting required.
• Requires working under occasionally stressful conditions to meet deadlines and patient/family needs.
• Must be able to make quick decisions, function as a facilitator to meet patient/family spiritual needs.
