Job Overview
The Social Worker facilitates treatment and provides coordination of referrals and intakes, direct practice in the areas of group work, consumer education, treatment planning and individual counseling. Participates as a member of the multidisciplinary treatment team, performing intakes, assessments, initiating treatment interventions and crisis intervention. Performs clinical case management and treatment planning with consumers that experience psychiatric disorders and substance abuse disorders.
All clinical documentation is completed with compliance in the requirements set by the NJ Division of Mental Health Services and the Joint Commission on Accreditation of Health Care Organizations. Participates in staff training and continuing education. Takes an active role in Performance Improvement projects. Is responsible for reporting identified safety issues such as hazardous environments and medical errors.
- Coordinate referrals and intakes from PESS, IP, PHP, State Psychiatric Hospitals, Community and other hospital systems including scheduling and tracking the status of referrals.
- Assist with psychosocial assessments, group notes, progress notes and treatment summaries according to policy.
- Facilitate patient education through individual and group counseling; prepare education materials; provide psycho-education for inpatients and outpatients; support patients and families prior to and after discharge.
- Manage intake coordination including intake line referrals, scheduling, and ensuring full schedules for Social Work Intake Assessment Providers; coordinate community and inpatient intakes and arrange medical appointments.
- Refer clients to other service agencies and assist with obtaining entitlements (e.g., Social Security Benefits, residential programs, Self Help).
- Coordinate with inpatient unit: complete assigned consults within 48 hours and participate in alternate treatment team meetings.
- Organize work with minimal supervision and prioritize tasks; make timely decisions aligned with program objectives, policies, and procedures.
- Participate in QA/PI efforts, complete QA/PI monitors, and follow up on action plans; provide recommendations for improvement.
- Provide a compassionate and supportive environment with prompt and consistent service; demonstrate strong interpersonal, communication, organizational and time-management skills.
- Perform inpatient engagement sessions to educate and link patients to follow-up services; perform intake assessments and present assessments to the treatment team; complete required clinical documentation.
- Conduct crisis intervention with the treatment team; collaborate with Screening Center staff when required.
- Master’s Degree in Social Work or related mental health service discipline
- Current NJ LSW or LAC licensure
- Experience providing individual and group therapy, intake assessments, treatment planning and crisis intervention
