Licensed Social Worker – Hospice Liaison
Location: Monmouth County
Schedule: Full Time
Overview: VNA Health Group is seeking a compassionate Licensed Social Worker (LSW) to join our Hospice Team. The Hospice Liaison plays a key role in assessing patients for hospice eligibility, coordinating smooth transitions of care, and providing emotional and psychosocial support to patients and families.
Responsibilities:
- Conduct hospice assessments and admissions in home and facility settings.
- Educate patients, families, and community partners about hospice philosophy and services.
- Provide emotional support and counseling during the referral and admission process.
- Collaborate with physicians, discharge planners, and the interdisciplinary team to ensure continuity of care.
- Participate in outreach and education to increase awareness of hospice services.
Requirements:
- Master’s Degree in Social Work (MSW) required
- Active New Jersey LSW license required
- Minimum 2 years of experience in healthcare, hospice, or homecare
- Strong communication, counseling, and organizational skills
- Valid NJ driver’s license, vehicle, and proof of insurance

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
