4/2 ROTATION SCHEDULE SHIFT, 3-11 (PM) AVAILABLE
FULL TIME

Responsibilities
  • Provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors.
  • Identify residents before administering treatments, serving meals, etc., using wristband or photo card.
  • Record information using only authorized abbreviations established by the facility.
  • Report all changes in a resident’s condition to the Nurse Supervisor/Charge Nurse promptly.
  • Record entries on flow sheets, notes, charts in an informative, descriptive manner.
  • Report all accidents and incidents observed during the shift.
  • Maintain confidentiality of residents’ protected health information and report violations to the Administrator.
  • Report any unauthorized attempts to access the facility’s information system.
  • Perform only nursing care procedures you have been trained to perform.
  • Ensure resident rooms are ready for admission (bed made, name tags up, admission kit available, etc.).
  • Greet residents and escort them to their rooms; introduce residents to roommates and other residents and personnel as appropriate.
  • Make residents comfortable; inventory and mark personal possessions as instructed; store clothing.
  • Assist residents with packing when transferring or being discharged; transport residents to new rooms or the receiving area; assist with loading/unloading residents to/from vehicles as necessary.
  • Create and maintain a warm, personal, and calm environment throughout the unit and shift.
  • Meet with shift nursing personnel to identify and correct problem areas and improve services.
  • Report complaints and grievances; participate in and receive the nursing report as instructed.
  • Follow policies concerning exposure to blood/body fluids and make beds as instructed.
  • Record temperatures, pulses, and respirations (TPRs) as instructed; weigh and measure residents as instructed.
  • Ensure residents unable to call for help are checked frequently; answer calls promptly; check personal care needs regularly.
  • Assist residents with identifying food arrangements and record resident food/fluid intake; report changes to Nurse Supervisor/Charge Nurse.
  • Keep water pitchers clean and filled; perform after-meal care (remove trays, clean hands/face/clothing).
  • Participate in and receive nursing reports; perform tasks per established policies and procedures and supervisor instructions.
  • Cooperate with inter-departmental and facility personnel to maintain nursing services; notify about lateness or absence.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policies.
  • Assist residents with daily functions (dental care, bathing, grooming, dressing, undressing); keep residents dry and comfortable.
  • Assist with preparing residents for medical tests; assist with lifting, turning, moving, positioning, and transporting as needed.
  • Perform restorative and rehabilitative procedures as instructed; prepare residents for physical examinations; monitor for pressure areas/skin breakdowns.
  • Report injuries of unknown source and observe/disoriented or comatose residents as instructed; perform special treatments as instructed.
  • Immediately notify Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility; use only trained equipment; report defective equipment.
  • Maintain confidentiality of all resident care information and report unauthorized disclosures; report all allegations of resident abuse or misappropriation of resident property.
  • Honor residents’ refusals of treatment and report such requests to your supervisor.
Supervisory Requirements

This position has no supervisor responsibilities.

Qualifications
  • Education and/or

    Experience:

    Must possess, as a minimum, a 10th grade education.
  • Language

    Skills:

    Ability to read, analyze, and interpret general business materials; write reports and procedures; present information effectively.
  • Mathematical

    Skills:

    Ability to apply fractions, percentages, ratios, and proportions to practical situations.
