Join a dedicated team at a permanent supportive housing site in Long Beach, where you’ll make a real difference in the lives of formerly homeless adults. We’re seeking an Intensive Case Manager who is passionate, resilient, and ready to support residents on their journey to stability and self‑sufficiency.
Key Responsibilities- Manage a caseload of 20–25 residents, providing intensive, individualized case management
- Develop and implement care plans focused on healthcare, mental health, behavioral health, budgeting, and life skills
- Meet with clients at least twice monthly, building trust and supporting long‑term recovery
- Coordinate access to community resources (DPSS, SSI, food banks, etc.) and advocate for client needs
- Set healthy boundaries while remaining responsive to urgent and impulsive client requests
- Support clients in developing routines, social skills, and self‑advocacy
- Collaborate with a small on‑site team and participate in ongoing training
- Confidence and high capacity to manage on‑site demands and frequent interruptions
- A minimum of 2 years of experience working with vulnerable populations, especially in housing or social services
- Excellent communication, organization, and problem‑solving skills
- Ability to set boundaries and maintain composure in a dynamic environment
- Education in social work or related field is a plus, but not required
Location: Long Beach, CA 90804 | Full‑time, Tuesday–Saturday 7:30a‑4:30p and 8:30a‑5:30p
Pay: $46,000.00/yr - $49,900.00/yr
