As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes, record clinical and progress notes, and coordinate other patient care activities to deliver the highest quality of patient care in your assigned geographic service area.

• Provide high‑quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other members of the health care team, following all applicable infection control procedures.
• Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency’s electronic visit verification program (as applicable).
• Actively participate in the agency’s care coordination process, including timely reporting of patients’ needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, and case coordinator or community case manager (as applicable).

• Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

• Home care experience.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.

Pay is based on several factors including, but not limited to, local labor markets, education, work experience, and certifications. The hourly pay for this role ranges from  20.38 to  36.44 per hour for full‑time employment.
