Exeter, Brentwood, Stratham, Hampton, Hampton Falls, Portsmouth, Rye, New Market, Epping, Freemont.
- Conduct thorough assessments of patients' psychosocial status in relation to their illness and environment. Communicate assessment findings to the registered nurse to ensure a holistic approach to patient care.
- Carry out social evaluations and develop effective intervention plans based on assessment findings. These interventions will contribute to enhancing the overall patient experience and care outcomes.
- Maintain accurate and up-to-date clinical records for all patients referred to social work, ensuring continuity and quality of care.
- Provide valuable information and referral services to patients, families, and caregivers concerning practical and environmental needs, fostering a supportive and informed healthcare environment.
- Act as a liaison between patients or families/caregivers and community agencies, building collaborative relationships to leverage additional resources for the benefit of patients and organization personnel.
- Work closely with the interdisciplinary team to develop comprehensive plans of care and participate in case conferences as required, facilitating seamless patient transitions and continuity of care.
- Assist physicians and other team members in understanding significant social and emotional factors that impact patients' health problems, contributing to a patient-centered care approach.
- Participate in discharge planning, ensuring patients receive appropriate support and resources after leaving the care setting.
- Holds a master's degree from a school of social work accredited by the Council on Social Work Education, showcasing a strong educational foundation for this role.
- Holds a valid license as an LICSW in good standing, demonstrating expertise and adherence to professional standards.
- Possesses a minimum of one (1) years’ experience in health care, with preference given to candidates experienced in home health care.
- Demonstrates excellent verbal and written communication skills, along with strong organizational abilities to manage caseloads effectively.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
- Pay Structure: SWs are paid per visit ($60), per call ($30), and per admission ($75). Training and meetings are paid hourly at $35/hr.
