Personal Advisor - Care Leavers

Location: Hybrid / Community-based
Contract: Full-time
Salary: Competitive (dependent on experience)

We are seeking a committed and compassionate Personal Advisor to support care-experienced young people as they transition into adulthood. This is a rewarding opportunity to make a tangible difference by providing practical, emotional, and aspirational support to young people aged 16-25.

The Role

You will work directly with care leavers to help them build independence, resilience, and positive futures. Managing your own caseload, you will provide tailored support across education, employment, accommodation, health, and wellbeing, ensuring each young person receives the right support at the right time.

Key Responsibilities
  • Deliver high-quality, person-centred support to care leavers to achieve positive outcomes
  • Prepare, review, and implement Pathway Plans in line with assessed needs
  • Coordinate services and work in partnership with families, carers, and external agencies
  • Support young people into education, training, and employment
  • Assist with accessing and sustaining suitable accommodation and tenancies
  • Support access to welfare benefits and financial entitlements
  • Maintain accurate, timely case records using electronic systems
  • Work holistically with young people with complex needs, including mental health, disabilities, parenting responsibilities, or immigration matters
  • Participate in duty rotas, team meetings, supervision, and service development activities
  • Work flexibly, including occasional out-of-hours work and visits outside the local area
About You
  • Experience working with care-experienced young people or vulnerable adolescents
  • Strong understanding of safeguarding and young people's legislation
  • Ability to manage a varied caseload and work autonomously
  • Excellent communication, organisation, and partnership-working skills
  • A proactive, empathetic, and solution-focused approach
  • Confident using IT systems for case recording and reporting
