Liquid Personnel is currently recruiting for an Adult Social Worker to join a Reablement Team based in Southwark.

About the role

Seeking a Social Worker to work within the Reablement Service. This role offers a unique opportunity to thrive in a multi-disciplinary environment, collaborating closely with health professionals.

What will your responsibilities be?

  • Goal setting
  • Completing Care Act assessments and devising care and support plans
  • Completing reviews
  • Completing Mental Capacity Assessments
  • Chairing Best Interests meetings

    • Qualifications and Experience:

    To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a Social Work degree, and have post-qualified experience.
