Oak Lodge Medical Center is seeking a dedicated and enthusiastic Practice Nurse to join our friendly and forward‑thinking team. We are a well‑established, high‑performing practice located in West Barnet, caring for a diverse patient population of approximately 17,000.

Main duties of the job

To provide high‑quality, evidence‑based nursing care to the practice population, with a particular focus on the prevention, early detection and management of long‑term conditions (LTCs) such as diabetes, COPD, asthma, cardiovascular disease and hypertension.

The postholder will:

  • Deliver nurse‑led clinics for LTCs in line with national and local guidelines (e.g. NICE, local ICB pathways).
  • Support achievement of QOF, IIF and other quality/improvement indicators.
  • Work as part of the multidisciplinary team (MDT) within the practice and PCN to improve outcomes and reduce health inequalities.
  • Contribute to proactive, population‑based care, including recall systems and structured reviews.

1. Job Purpose

To provide high‑quality, evidence‑based nursing care to the practice population, with a particular focus on the

2. Key Duties and Responsibilities

2.1 Clinical Care Long‑Term Conditions

  • Undertake
    structured annual and interim reviews
    for patients with LTCs (e.g. diabetes, COPD, asthma, CHD, heart failure, hypertension, CKD), in line with practice protocols and national guidance.
  • Perform and interpret relevant clinical measurements, for example:
  • Blood pressure, pulse, BMI, peak flow/spirometry (if trained and accredited), foot checks, urinalysis, blood glucose monitoring, etc.
  • Use templates and recall systems to ensure
    systematic, proactive management
    of LTC registers.
  • Provide
    individualised self‑management support and education
    , including lifestyle advice (smoking cessation, weight management, physical activity, diet, alcohol).
  • Identify patients at risk of deterioration or complications and
    escalate to a GP or appropriate clinician
    as per practice protocols.
  • Contribute to
    medication monitoring
    (e.g. BP checks, inhaler technique, insulin administration teaching, adherence support) under the direction of a prescriber, without independently initiating or altering medication regimes.
  • Support delivery of
    anticipatory and proactive care
    for frail and high‑risk patients as part of PCN/ICS pathways.

2.2 General Practice Nursing

  • Provide a full range of treatment room and practice nursing services, which may include:
  • Wound care and dressings
  • Cervical cytology (if trained and accredited)
