Job overview

We have a vacancy in Hillingdon for a full‑time Band 5 community staff nurse in the School Nursing Service. The Service delivers an innovative and progressive approach to school nursing and works closely with schools, Children’s Centres, Family Hubs and other local agencies. Applicants should be enthusiastic about improving nurse‑led care for children and young people.

Main duties of the job

You will work autonomously as part of a multi‑skill team, be flexible and responsive, and contribute to modernising practice and influencing health and Trust policies. You will use your interpersonal skills to identify low protective factors, abuse, neglect and mental health concerns in young people, develop strength‑based plans of care, and promote resilience and wellbeing.

Detailed job description and main responsibilities

Clinical Responsibilities
  • Deliver the Healthy Child Programme.
  • Undertake holistic health needs assessments of children in diverse environments and across a broad population spectrum.
  • Implement a plan of care using evidence‑based criteria to rationalise professional decisions and actions, monitoring progress and adapting plans to maximise health benefits.
  • Provide skilled, evidence‑based nursing care in line with agreed policies and procedures.
  • Provide ongoing support for individuals and families, sometimes delegated by senior staff, and report back to senior colleagues.
  • Signpost and refer to specialist services for support and intervention.
  • Help children, parents and carers identify their own level of knowledge and skill, offering support, information and advice to enable healthy lifestyles.
  • Communicate effectively with children, parents/carers and other professionals using excellent communication skills.
  • Maintain accurate and comprehensive record keeping in line with NMC guidance and Trust policy.
  • Have knowledge of Information Sharing and Assessment protocols and the ability to identify relevant information to promote child safety.
Professional Responsibilities
  • Use a range of skills to communicate, taking account of culture, ethnicity, language, disability, developmental age and setting.
  • Engage effectively with children and families, social care, education, colleagues and other professionals.
  • Manage all records in line with the Trust Health Records Policy and relevant guidelines.
  • Maintain knowledge of key IT systems (Outlook, Systm One) and ensure proficiency through training.
  • Apply Infection Control Procedures and implement recommendations as necessary.
  • Participate in team discussions and support service developments.
  • Maintain up‑to‑date NMC registration and adhere to the NMC Code of Conduct.
  • Critically reflect on own performance through clinical supervision.
  • Contribute to personal development plans, 1:1 meetings and appraisals.
  • Ensure mandatory training is updated annually or as specified.
  • Be aware of children’s safeguarding policy, attend safeguarding meetings and supervision, and report concerns to the Team Leader School Nurse.
  • Monitor practice and pursue continuing professional development.
  • Recognise personal distress and seek support when faced with highly emotive circumstances.
  • Act as a role model, communicate well with the team, understand others’ roles, and encourage reflective practice.
  • Encourage learning from adverse events, audit data and implement sustainable improvement initiatives.
  • Work confidently as an autonomous practitioner and support others, including less experienced staff.
