Job Summary

Sevenfields Primary Care Network (PCN) seeks a self‑motivated and experienced Advanced Nurse Practitioner to join our multidisciplinary team under the Additional Roles Reimbursement Scheme (ARRS). The role is to provide an acute home visiting service, assessing housebound patients presenting with acute illness and injuries, as well as performing long‑term condition reviews for this subset of patients as required by the four practices within the PCN, serving a combined population of approximately 41,000 patients.

About

Us

Sevenfields PCN is embedded into its community with active link workers and a staff base at Goldsmiths Community Centre which also offers vaccinations and other services. We have a real passion for the health and wellbeing of our residents and have invested in four outdoor gyms for our local communities using surpluses generated by our vaccination work which was largely delivered by our active volunteer team.

Our local population is somewhat older than other parts of Lewisham and contains several large care homes. It includes areas of extreme socioeconomic deprivation and the PCN is keen to find innovative ways of addressing the health challenges that accompany this. All of the GP practices in Sevenfields are shareholders (members) of our PCN Company which has a Board that provides clear leadership through strong GP practice representation.
