Job Summary

We are seeking a highly motivated and forward-thinking Practice Nurse to join our innovative, training-focused practice serving 20,000 patients in Wallington. The practice is committed to delivering high quality patient care which requires a supportive and cohesive team. We require a skilled and enthusiastic nurse with chronic disease experience to join our nursing team. The position is available for an immediate start.

About

Us

Wallington Family Practice currently provides general medical services to a population of approximately 20,000 patients. It has 11 GPs, a team of pharmacists including pharmacy technicians, a lead nurse, 5 practice nurses, 1 HCA, and an extensive team of admin and reception staff. We are a large proactive teaching practice located in a purpose-built health centre close to Wallington town centre and are part of the Wallington Primary Care Network (PCN).

The main purpose of this role is to provide efficient, courteous and responsive care navigation to the diverse needs of our patients.

Job Responsibilities
  • Work autonomously within the multi-disciplinary team to provide a full range of Practice Nursing services, including cervical cytology, chronic disease management (as qualified), patient education, health promotion, wound management, ear irrigation, contraception advice, vaccinations, travel health vaccination and advice, and other general treatment room services as required.
  • Provide assessment, implementation and evaluation of clinical packages of care.
  • Advise patients and other clinical staff on management of chronic diseases and appropriate therapies.
  • Refer patients independently to other clinical disciplines internally and externally.
  • Undertake clinical audit activities to review care provided, make recommendations for improvement, and implement agreed changes.
  • Assist with QOF and other target work to ensure maximum performance for the practice.
Administrative Duties
  • Keep accurate and contemporaneous notes of consultations on the computer system.
  • Maintain and monitor medical supplies, order and record supplies from appropriate suppliers, and ensure appropriate storage upon receipt.
  • Ensure regular routine servicing and maintenance of medical equipment and keep records of repairs.
  • Ensure clinical rooms are well stocked and maintained.
  • Assist with recall or follow-up systems to encourage patient attendance.
Communication
  • Establish and maintain effective working relationships with all members of the clinical and non-clinical team.
  • Communicate effectively with patients and their relatives through verbal and non-verbal means.
  • Assess patients’ ability to give informed consent and seek assistance if unsure.
  • Read and reply to internal and external emails on a regular basis.
Confidentiality and Data Protection

The post holder will have access to sensitive patient information. Staff must respect patients’ privacy, maintain confidentiality, and act appropriately. Information must be disclosed to an appropriate clinician if the patient is at risk of harm to themselves or others in accordance with the practice’s policies.

  • Assist in managing junior nursing staff and provide mentoring and supervision.

  • Identify new opportunities for service development and manage clinical and non-clinical time of staff.
  • Support training and mentoring of medical and nursing students when required.
