PELC is seeking an enthusiastic Advanced Clinical Practitioner or Advanced Nurse Practitioner with independent prescribing to work with us across our sites. You will deliver face‑to‑face services, enabling patients to obtain health information from the appropriate health care professional. You will be providing safe and effective health assessments and treatment of patients within the establishment.
You will be accountable for your practice including the assessment, diagnosis, treatment, and discharge or referral of patients, independent of medical intervention to provide high‑quality support for professional practice within the organisation.
Main duties of the jobThe post holder will be an experienced caring nurse/practitioner who can demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries. You will work within a clinical team consisting of GPs, UCPs, Nurse Streamers and HCAs to deliver excellent care services within our UTC. You will communicate at all levels across boundaries and work closely with wider clinical team members.
The post holder will also work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures.
You will be an integral member of the clinical team, completing streaming, independently assessing patients and prescribing clinical intervention. You will also be completing administrative tasks (prescriptions, referrals, pathology results).
As an ANP/ACP you will be supported by, and work with, a diverse multi‑skilled professional team of clinical and non‑clinical friendly staff, providing high quality, person‑centred, holistic clinical care to patients.
Clinical practiceAssess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug‑ and non‑drug‑based treatment methods into a management plan.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence‑based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over‑the‑counter medication on medication regimens, side‑effects and interactions.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self‑care.
Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
Promote and deliver evidence‑based care for patients presenting for opportunistic wound care.
CommunicationUtilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
Anticipate barriers to communication and take action to improve communication.
Maintain effective communication within the practice environment and with external stakeholders.
Act as an advocate for patients and colleagues.
Delivering a quality serviceRecognise and work within own competence and appropriate professional code of conduct as regulated such as NMC and HCPC.
