Job SummaryThe Community Palliative Care Specialist Nurse will work to empower people with advanced progressive illnesses and those who support them to maximise their individual quality of life and death by enabling the provision of skilled palliative care.
This role within an established team will support ongoing development of the post holder, giving them the opportunity to advance their palliative and end of life care skills alongside a multi‑professional team.
Key Working RelationshipsSpecialist Palliative Care services in Harrow and Northwest London, St Luke's Hospice, acute based Palliative Care and Macmillan services, General practitioners, adult community service providers including other specialist teams and district nursing, social services.
Main Duties of the JobThe CNS in Palliative Care works as a member of Harrow Community services within Central London Community Healthcare NHS Trust. The team is comprised of a clinical lead, nurse team leader, clinical nurse specialists, palliative nurse, 3 part‑time consultants in Palliative Medicine and 1.2 WTE team administrators.
Acting as a specialist resource to provide advice, support and information to patients, families and staff, the CNS plays a key role in raising clinical standards in the provision of palliative care services in the community. The CNS will act as a role model and exercise leadership to influence and develop the practice of nurses, medical and social care personnel in palliative and end‑of‑life care.
In collaboration with the Palliative Care Consultants and other members of the specialist multi‑disciplinary team (MDT) the post holder will provide expert advice on pain, symptom control and the ongoing management of patients with specialist palliative care needs.

Person Specification Education / Qualification- Essential:
Registered Nurse (RN), NMC Registration - Desirable:
Evidence of other post‑registration education and training
- Recent clinical nursing experience negotiating and liaising within a multi‑disciplinary team
- Working with people with complex health needs
- Supporting students and other learners e.g. HCAs teaching others in the practice environment
- Experience of undertaking general assessment using evidence‑based care
- Previous experience in the Nursing process, care planning and fluent record keeping
- Experience of providing holistic care involving the multidisciplinary team
- Desirable:

Participation in audit, research projects, service development - Desirable:
Post‑registration experience in an NHS community environment
- Knowledge of out‑of‑hospital care and the role of the Community Nurse
